In order to complete this application, you MUST submit proof of studio affiliation via file upload. Please gather the appropriate documentation before beginning your application.

We sell exclusively to dance studio owners and dance team directors. In order to become an A Wish Come True customer, you will need to complete and submit the online form below and provide one of the following for proof of your studio affiliation:

  • Pre-printed bank deposit slip with studio name and address
  • Pre-printed voided business check with studio name and address
  • Business license
  • Business lease
  • Dance team directors, please provide a letter stating your position on school letterhead, with the signature of the principal or athletic director
  • Australian and New Zealand studios, please include your ABN or NZBN documentation
